Abstract:
This dissertation consists of three parts. Both part one and two each made up
of 10 obstetrical and 10 gynaecological cases and commentaries respectively.
All these 20 case reports and commentaries were managed by the author under
supervision of senior staff members in the department. For each case a thorough
history, examination, investigations, and treatment have been narrated. Cases were
managed according to standard management protocols, though investigations at times
were not exhaustive because of limited facilities. After each case there is a
commentary followed by relevant references.
The third part consists of research paper on Fetal movement count and
outcome of pregnancy at Muhimbili Medical Center, Tanzania. This was an
observational descriptive study. The study described the use of fetal movement count
for surveillance of high risk pregnancies. The mean gestation age for fetal movement
count was 34.8 weeks. Patients with reduced fetal movement count were 24 (16.9%)
and out of these 22 (91.6%) were delivered by intervention. There were 5 (20.8%)
still births and 2 (8.7%) had severe birth asphyxia. This was lower than in the group
with no reported reduced fetal movements 16 (13.6%). The FMC was able to pick
the most severely forms of birth asphyxia, and the results were statistically
significant. p = 0.03.
