The effects of controlled ventilation by the Manley Multivent Ventilator on the cardiopulmonary stability in patients who underwent various surgical procedures under balanced anaesthesia, were studied in 102 patients of American Society of Anaesthesiologists (ASA) class 1 and 2. 'The general performance of the ventilator and complications arising from its use were also assessed. Controlled ventilation by intermittent positive pressure ventilation (IPPV) was found to have no significant effect (P > 0.10) on the blood pressures of the patients. The pulse rate changes were also not significant (P > 0.05) i and a good peripheral oxygen saturation of 96 to 100% was maintained in all patients. The Multivent performed satisfactorily to provide effective ventilation using maximum inspiratory pressures of 20 to 35cmH20, tidal volumes of 6.7 to 12.5mls/kg body wt., respiratory rate 12 or 14 cycles/min and I:E ratio of 1:2. These parameters were regulated and adjusted successfully according to individual patient requirements, and adequate and safe ventilation was achieved. The ventilator was simple to operate, versatile, reliable and safe for use during anaesthesia and no complication was associated with its use. The Multivent with its unique features of flexibility in both power and gas supply, coupled with its low purchasing iii and running costs, provides anesthetist in the developing world with an economical, reliable and safe means of controlled ventilation.en_GB
